Enclose a check for $260, payable to Mountain Book Company. This covers your one-time set up fee of $200, plus your first annual fee of $60. Mail to PO Box 778, Broomfield, CO 80038-0778. If Mountain does not accept your application, the check will be returned.

I, the undersigned, have read the information on Mountain Book Company's website and would like them to receive EDI purchase orders for our books on our behalf. I understand Mountain will invoice bookstores, email shipping instructions to me, collect money from purchasers, and after receiving payment from the stores, pay me fifty percent of the retail cost of my books, less document fees charged by Pubnet (now at 75 cents per document). I agree to ship books promptly via a carrier that provides shipment tracking. If I use Mountain's shipping account, they will deduct shipping costs prior to paying me. This agreement shall be for one year, renewable by mutual consent. I understand that if I fail to ship books promptly, or in any other way act in a manner detrimental to Mountain or its other publishers, Mountain may, at its sole discretion, terminate this agreement and any moneys I have paid will be forfeit. Mountain agrees not to exercise this right without reasonable cause.
